PREPS YOUTH FOOTBALL LEAGUE

ONLINE WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK, MEDICAL AUTHORIZATION, AND MEDIA RELEASE

Preps Youth Football League (PYFL) is a 501(c)(3) nonprofit organization.

By checking the "I Agree" box during registration, I certify that I am the parent or legal guardian of the participant and that I have read, understood, and agree to the following terms:

ASSUMPTION OF RISK

I understand that participation in football, flag football, conditioning, practices, games, tournaments, camps, clinics, and related activities involves inherent risks, including but not limited to cuts, bruises, sprains, fractures, concussions, serious injury, permanent disability, illness, and death.

I voluntarily allow my child to participate in PYFL activities and knowingly assume all risks associated with participation.

RELEASE OF LIABILITY

To the fullest extent permitted by New Mexico law, I release and hold harmless Preps Youth Football League (PYFL), its officers, directors, board members, employees, volunteers, coaches, assistant coaches, officials, sponsors, agents, affiliates, facility owners, and event organizers from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising from or related to my child's participation in PYFL activities.

This release includes claims arising from practices, games, tournaments, camps, clinics, league events, and related activities, including those resulting from ordinary negligence.

MEDICAL AUTHORIZATION

I certify that my child is physically able to participate in athletic activities and that I have disclosed any known medical conditions during registration.

In the event of an injury or medical emergency, I authorize PYFL representatives, coaches, volunteers, and emergency medical personnel to obtain emergency medical treatment for my child when I cannot be reached. I understand that I am solely responsible for any medical expenses incurred.

CONCUSSION ACKNOWLEDGMENT

I understand that football is a contact sport and that concussions and head injuries are potential risks. I agree to comply with all league safety policies, including concussion protocols, return-to-play requirements, and medical clearance procedures.

FACILITIES AND TRANSPORTATION

I understand that PYFL cannot guarantee the condition or safety of fields, facilities, parking areas, equipment, or surrounding property. I acknowledge that transportation to and from league activities is my responsibility unless otherwise provided by PYFL.

CODE OF CONDUCT

I understand that participants and parents are expected to follow all league rules, policies, and instructions from coaches, officials, and league representatives. PYFL reserves the right to suspend or remove any participant or spectator whose conduct is deemed unsafe, disruptive, abusive, or detrimental to the league.

PHOTO AND MEDIA RELEASE

I grant PYFL permission to photograph, videotape, livestream, and record my child during league activities. I authorize PYFL to use such photographs, videos, recordings, and likenesses for promotional, educational, fundraising, website, social media, and marketing purposes without compensation.

INDEMNIFICATION

I agree to indemnify and hold harmless PYFL from any claims, damages, liabilities, costs, or expenses arising from my child's participation in league activities or from violations of league rules and policies.

ACKNOWLEDGMENT

By selecting "I Agree," I acknowledge that I have carefully read this waiver, fully understand its contents, understand that I am giving up certain legal rights on behalf of myself and my child, and voluntarily agree to all terms and conditions contained herein.
