About the form
This registration form may be used for up to five individuals. 
A separate form must be completed for each day of play. 
Every attempt will be made to assign all people on the form to the same team, but not guaranteed.
DUES
Each player is responsible to pay the $10 registration fee to their TEAM CAPTAIN once assigned to a team. 
Players can play on multiple days but must pay a registration fee for each league day.
Other Information
All new players and any players that have not played since Spring 2020 MUST complete a new waiver. 
Also all players must complete a Code of Conduct form if you have not already completed one.
Forms may be printed from the Downloads tab on this website.  All required forms shall be submitted with your team dues payment to the team captain. 
A player is not eligible to play until both forms are completed and submitted to registrar.
All players must adhere to PC Bocce Ball Code of Conduct.
Once all the information required has been entered into the form, please hit the Submit button at the bottom of the form.  
Do not print and mail the registration form. 
Phone numbers shall be in XXX-XXX-XXXX format.
