The following form is for individual registration for Faith Baptist Youth Soccer League for FALL 2025
Individual Player Fee is $110
(early bird pricing is $95)
Team Fee is $350
Ages as of June 1, 2026
Games will be played at Buckhead Creek Recreation Complex
Age group divisions will be determined at the end of registration based on the quantity of players but usually divided like this:
6U
-4/5/young 6s (4v4) 6 teams with 7-8 players on roster
8U
-older 6s/7/8 (5v5) 6 teams with 9-10 players on roster; 54-60 player spots
11U
-9/10/11 (8v8) 4 teams with 12-13 players on roster; 48-54 player spots
Teams are coed.
There are no refunds after August 8
Practices will begin the early August and games begin in September after Labor Dayl. Season ends late October
*Games will be played on Saturday mornings and Tuesday evenings or Thursday evenings-any combo of those days.
Practices will be determined by the coaches and will be up to 2 practices a week. Practice days could be Mondays, Tuesdays, Thursdays, and/or Saturdays. 
Rain out games will be built into the schedule. We will schedule 8-9 games but hope for 6 games. 
Practice is expected to start the Week of August 8, 2026. No practices on Labor Day Weekend September 5 or Labor Day September 7
Games are expected to start April 18, 2026

Coaches can only use one code for 1 child in the family.

THE PARTICIPANT IS RESPONSIBLE FOR HIS OR HER OWN MEDICAL COVERAGE NOTICE OF WARNING: There is a potential risk in training and participating in any sport, and we have tried to create a safe environment. The coaches have established rules for participation, and proper conduct on or about the playing field must be followed. You are responsible for your own medical coverage.

EMERGENCY AUTHORIZATION:

I hereby authorize the coaches, assistant coaches or team members acting in the capacity of activity supervisors/vehicle drivers, as my Agents, to consent to medical, surgical or dental examination and/or treatment.  In case of emergency I hereby authorize treatment, and/or care at any hospital. 

Wavier of Liability:I have read and understand the policies and the risk involved. I hereby agree to follow all rules for good order and safety during this season. I agree and understand that neither Faith Baptist Church, Buckhead, nor any of the workers/volunteers involved are liable for any injuries received while participating or playing in the activity for which you are registering herein, or for the loss or damage to equipment. I agree that I shall make no claim and bring no action, suit, or proceeding for any and all damages, losses, liabilities, or costs in any manner suffered or incurred as a result of my participating in the activity for which I am registering herein, and I hereby release Faith Baptist Church from any and all damages, liabilities, or costs in this regard.

PHOTOGRAPHS AND VIDEO: I understand that any pictures or video footage taken by church personnel may be used In future promotion material for the church league. Please contact so@myfaithbaptist.org if you do not agree to the media release statement.
