WAIVER AND RELEASE
STANTON PARKS & REC SPORTS LEAGUE
The undersigned is desirous of participating in the Stanton City Park Basketball. Baseball, Soccer, Flag Football, Corn Hole, Softball, Pickleball, Tennis, Dodgeball, Volleyball, or T-Ball League, (hereafter referred to as “the League”), with the games being played and the accompanying events being held at the Stanton City Park, Stanton Christian Church, Powell County High School, Powell County Middle School, Stanton Elementary School, Bowen Elementary School, Clay City Elementary School, or Emmanual Baptist Church.
The undersigned by the execution of this waiver and release acknowledges that there exists both inherent and unforeseen risks to the undersigned and others as a result of engaging in the above-described activity, (league play), and the undersigned hereby states that the undersigned knowingly and voluntarily chooses to participate in the league and assumes all the risks associated with play in the league, both to myself and to my property. I acknowledge that the above activity may be physically strenuous and that I know of no medical or other reason why I should not participate. I knowingly and freely assume all risks, both known and unknown, and assume full responsibility for my participation.
I agree to comply with the stated and customary rules for participation. If I observe any unusual significant hazard during my participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.
I, for myself, my heirs, personal representatives and assigns, do hereby release, waive, covenant not to sue, indemnify and agree to hold harmless for any and all purposes the City of Stanton, its elected representatives, officers, officials, agents, volunteers and employees from any and all liabilities, claims, demands, injuries or damages including any court costs, attorney fees and expenses which may occur to myself, other participants, and third parties as a result of my participation and conduct in the basketball league and with activities connected with the league while on the premises or otherwise. Undersigned agrees to not hold the Stanton Christian Church responsible for any accident or loss which may occur on the premises.
League games will be held at the Stanton City Park, Stanton Christian Church, Powell County High School, Powell County Middle School, Stanton Elementary School, Bowen Elementary School, Clay City Elementary School, or Emmanual Baptist Church. The undersigned agrees to conduct themselves in a proper manner and to be respective of others and of the premises. When at the Stanton City Park, the undersigned agrees to abide by the posted rules for the Stanton City Park and all laws and ordinances pertaining to the Stanton City Park. When at the Powell County High School, Powell County Middle School, Stanton Elementary School, Bowen Elementary School, or Clay City Elementary School, the undersigned agrees to abide by the posted rules for that location and all laws and ordinances pertaining to the property of the Powell County School System. When at the Emmanual Baptist Church, the undersigned agrees to abide by the posted rules for the Emmanual Baptist Church. When at the Stanton Christian Church, the undersigned agrees to abide by all rules and guidelines established by the Stanton Christian Church including, (but not limited to), the following:
- Alcoholic beverages, foul language, obscene clothing, etc., as well as smoking on the premises is not permitted.
- Prayer will be conducted at the beginning of all events.
- No alcoholic beverages or tobacco products are permitted on the premises.
- Use of secular music is prohibited.
- An entry fee may not be charged at any event.
- No gambling or wagering is permitted on the premises.
- Premises are to be cleaned up prior to exiting the facility.
- Children are always to be supervised by an adult.
- Stanton Christian Church is not responsible for any accidents or injuries which may take place on the property or any loss of personal property.
- Individuals are to be respectful of other participants, officials, and others in the building at all times.

In signing this agreement, I acknowledge and represent that I have read it, understand it and sign it voluntarily as my own free act and deed. I have not relied on any oral representations or statements apart from the terms contained herein. I execute this document for full, adequate, and complete consideration of being able to participate in the League.
