VOLUNTEER COACH INFORMATION
1. During the registration process for your child mark "YES" to be the Head Coach.

2. Read the following documents: Concussion Policy (read only), Coach Checklist, Code of Conduct, Sportsmanship (read only)

3. Fill out and return to our office Coaching Right to Access Waiver & Risk Ack. You only need to fill out the second page.
4. All adult volunteers must schedule and pass a background check. You can stop by the Parks & Recreation Office to fill out the form. Our hours are Monday - Thursday 8:00 AM - 4:00 PM. Background checks are processed through the Mapleton Police Department. Please make sure to bring your drivers license.

Marking "YES" allows us to identify potential coaches. This is not a guarantee that you will coach.
