COACH POPE FLAG FOOTBALL CLINIC - November 11, 2026 WAIVER AND RELEASE OF LIABILITY
I waive and release Coach Pope Sports (and its affiliated/parent organizations, including Tribe Academy LLC, Courage Builder) and the clinic venue Wide World of Indoor Sports and anyone associated with the Coach Pope Flag Football Clinic taking place at Wide World of Indoor Sports North Kingstown, RI on November 11, 2026 from any and all liability from injury, accidents, and medical or dental expenses incurred as a result of participation at the clinic. I, as parent/guardian, have actual knowledge and appreciation of the particulars of the program, hereby voluntarily consent to said minors participation and assume the risk arising there from. I hereby give my permission for emergency medical treatment in the event I cannot be reached via the phone number listed below as the emergency contact.
In consideration with allowing the child whom I am considered to be the legal parent or guardian of, to participate in the COACH POPE FLAG FOOTBALL CLINIC, related events and activities, the undersigned acknowledges, appreciates, and agrees that:
The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child’s participation. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove my child from participation and bring such to the attention of the nearest league employee/official immediately.
By signing this agreement, I acknowledge the inherent risk of exposure to communicable diseases, including but not limited to COVID-19, influenza, and other viral or bacterial pathogens that may be present in public spaces. I voluntarily assume all risks related to such exposure and agree to hold the organization and its affiliated organizations and partners harmless from any liability, illness, injury, permanent disability or death resulting from your participation or presence at the facility.
I, for myself and on behalf of my child, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Coach Pope Sports, Tribe Academy LLC, Courage Builder, Wide World of Indoor Sports and other participants, sponsoring agencies, sponsors, advertisers, and owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
