I give my permission for my child to participate in basketball practices and games sponsored by Hoop Rat Basketball Club and games. I hereby state that my child is physically able to do the activities therein. I will not hold Hoop Rat Basketball Club, or any employee or volunteer thereof liable for any injuries that may result from my child’s participation. I hereby give my permission for emergency medical treatment in the event I cannot be reached.
Photo Waiver
I hereby consent that that the photographs and/or videotape may be used by Hoop Rat Basketball Club in electronic or printed media, such as posting photos or videos to Hoop Rat Basketball Club website or any social media site, or using my child’s photograph on newspaper stories, advertisements, and flyers.

Please note that this year's sessions will take place on Saturday's with the possibility of some Sunday sessions if Saturday gym time is unavailable. We will be working as hard as we can to notify participants of changes at the end of practice as well as a Facebook post so make sure to follow us and check in with your child for any updates.
Also, we are always looking for help with the program, if your child is part of the program at any level and you would like to help out, we could use help with checking kids in, helping keep kids in line and actively participating during practice, fundraising opportunities, Refs, scoreboards and concessions during the annual tournament, and possible Board seats. Please feel free to reach out to Sarah Dybdahl or Josclyn Ramirez if you would like to volunteer.
