WAIVER OF LIABILITY RELEASE FORM

 

 

 

I am aware of the activity involved and give permission for the above child(ren) to participate and to be photographed for publicity purposes. I understand that this completed form must be in the possession of the FORT BEND BASKETBALL ASSOCIATION Inc , Texas Premier AAU prior to participation in this program.  I do hereby waive, release and agree to hold harmless Missouri City Rec & Tennis Center , Fort Bend Basketball , Harmony School of Innovation, the league organization, league players, the organizers, sponsors, supervisors, coaches and participants for any claim arising out of injury as a result of participation. I also grant permission to managing personnel or other league representatives; to authorize and obtain medical care from any licensed physician, hospital or medical clinic should the player become ill or injured while neither parent nor guardian is available.

 

 I have carefully read this agreement waiver and release and fully understand its content. I am aware that this is a release of liability and a contract between the above entities and myself and I sign it of my free will.
