If paying in person, payment must be received by October 31, 2026, in order for participants to play. No payments will be accepted during evaluations, as payment must be finalized prior to that date.

I certify that all the information contained herein is correct, and I do give consent for my child's participation in the programs dealing with basketball by Godley Youth Basketball Association (GYBA). I hereby waive any claims against players, sponsors, or volunteers which may arise from any activities of GYBA. If my child is injured playing basketball or in connection with this sport, I hereby authorize the coach or manager of his/her team to obtain medical attention as he/she may need.
WAIVER/RELEASE FOR COMMUNICABLE DISEASES
ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT
In consideration of being allowed to participate on behalf of Godley Youth Basketball Association athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Godley Youth Basketball and their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

PARENT/SPECTATOR CODE OF CONDUCT
“This game is not about parents but the children and I will praise every child and make them all feel like winners”
I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game, practice, or youth sports event.
I will place the emotional and physical well being of my child ahead of a personal desire to win.
I will insist that my child play in a safe and healthy environment. I will inform the coach of any physical or mental disability that may affect the safety of my child or others.
I will learn the rules of the game and the policies of the association.
I will respect the decisions of the officials understanding their job is difficult, but could be made easier with cooperation and support from all parents and friends.
I will demand a sports environment for my child that is free of drugs, tobacco, and alcohol, and will refrain from their use at all youth sports events.
I will do my very best to make youth sports fun for my child and their teammates.
I will ask my child to treat other players, coaches, fans, and officials, with respect regardless of ability, sex, creed, or race.
I promise to help my child enjoy the youth sports experience by doing what I can to assist the team and coach, by bringing snacks or by providing transportation.
I will not engage in unsportsmanlike conduct with any official, coach, player or parent such as booing or taunting, refusing to shake hands or using profane language or gestures. I will ensure the same conduct of my guests.
I will refrain from coaching my child or other players during games and practices unless I am one of the official coaches of the team.

COACH/VOLUNTEER CODE OF CONDUCT
“Coaching is a Privilege not a Right and I will do everything within my power to be a positive role model for my players”
I hereby pledge that I will place the emotional and physical well being of my players ahead of my personal desire to win.
I will treat each player as an individual, remembering the large range of emotional and physical development for the same age group.
I will do my best to provide a safe playing situation for my players.
I will do my best to organize practices that are fun and challenging for all my players.
I will provide a sports environment for my team that is free of drugs, tobacco, and alcohol, and I will refrain from their use at all youth sports events.
I will be knowledgeable in the rules of each sport that I coach and the policies of the association.
I will remember that I am a youth sports coach, and that the game is for children and not adults.
I will never yell at or ridicule a player for making a mistake or losing a game nor will I allow it from others.
I will accept the decision of the officials. I will not engage in unsportsmanlike conduct with any official, coach, player or parent and will ensure the same conduct of my players and parents.
Profanity in any form will not be allowed at my practice or youth sports event.
I will establish objectives for my team and will encourage open communication between my players, coaches and parents.
I will play my players a fair amount of time and will not allow retribution in any form against another player, coach, parent or official.
I promise to review and practice the basic first aid principles needed to treat injuries of my players.
I will require that my team’s parents and players understand and have agreed to the GYBA Parent and Player Code of Ethics.

in regards to abusing officials, players, coaches and other fans.
This means:
No negative comments or displays of negative behavior directed towards officials, players, coaches or fans .
Please note: SPECTATORS WILL BE REMOVED FROM THE GYM WITHOUT ANY WARNING IF ANY NEGATIVE COMMENTS ARE DISPLAYED (VERBALLY OR PHYSICALLY) AND WILL NOT BE ALLOWED BACK IN ALL GYMS FOR THE REMAINDER OF THE DAY .
**This also includes Coaches berating officials in a manner that can be heard from the players benches.
We understand that attending or coaching these games can be frustrating at times but we also have to keep in mind that kids are here to learn how to play with integrity, fair-play, and sportsmanship. This also includes coaches, parents and spectators.
Coaches,
Please be aware that if you harass officials the following steps will occur:
1st warning - You will be asked to sit on the bench for the remainder of the game.
2nd warning - You will be removed from the bench and gym for the remainder of the day.
**If you happen to be the only coach on the bench when the game starts and you are removed from the games after the 2nd warning then your team will have to forfeit the game in which the violations occurred.
Payment
If you chose pay in person, your child's uniform will not be ordered and they will not be placed on a team until paid. (Uniform is required)
If Payment is not received by November 1st, 2025 by 8:30 p.m., your player will not be able to participate for the 25/26 season.
