Release of Liability
Waiver
Registration and waiver of claims: I, as a parent or legal guardian, hereby give permission for my child to participate in the Huffman Youth Volleyball League and acknowledge the fact that he/she is physically able to participate in any and all activities volleyball related. I hereby authorize the directors of Huffman Youth Volleyball League to act for me according to their best judgement in any emergency situation requiring medical attention, and acknowledge that we, the parent or legal guardian, will be responsible for any costs (through medical insurance or otherwise) incurred due to sickness or injury. I hereby waive any claim I might have against Huffman Youth Volleyball League and the institution providing access to these facilities.

I am the parent or legal guardian of the Participant. I have read this waiver, and I am signing it freely. I understand the legal consequences of signing this document, including (a) releasing Huffman Youth Volleyball League and Huffman ISD from all liability on my and the Participant’s behalf, (b) waiving my and the Participants’ right to sue Huffman Youth Volleyball League and Huffman ISD, (c) and assuming all risks of Participant’s participation in this activity or any events incidental to this activity. I allow the Participant to participate in this activity. I understand that I am responsible for the obligations and acts of the Participant.

Refund Policy
The Policy
All registration fees are due by August 18th. Unpaid players will not be assigned to a team. 
Refunds can be requested prior to player evaluations. After uniforms are ordered, refunds will be looked at on a case by case basis and may be subject to a percentage held for costs incurred. All refund requests should be sent to huffmanvolleyballleague@gmail.com
