*Weigh In Information
 I understand that weigh in is on July 29, 2026 from 12:00 PM - 4:00 PM and boxers that have not checked in by 4:00 PM will not be able to weigh in and will not be refunded. 
*Physical Information
 I understand all boxers registered must have a current physical that is good through August 2, 2026. Physical paperwork will be required at registration. 
*Ohio State Fair Tickets
 I understand that the Ohio State Fair is no longer providing complimentary (free) tickets for entry for coaches or participants. Each registered participant will receive 3 Ohio State Fair entry tickets included in registration (one for each day of the tournament). Coaches can purchase tickets at Registration for $7 if they are on a registration form. Parking is not provided. 
*Unopposed Boxer Information
 I understand that it is possible for a registered boxer to be unopposed. If unopposed, to claim award, boxer must attend the Ohio State Fair Boxing Tournament on Thursday July 30, 2026 or Friday July 31, 2026 at a time that will be communicated upon conclusion of the draft.. 
By submitting this form, you agree to: As a participant, or as a parent/guardian of the participant in this program, I recognize that there are certain risks of physical injury and I agree to assume the full risk of any injuries, damages, or loss resulting from participation in any and all activities connected with or associated with such program. I agree to waive and relinquish all claims I may have as a result of my son/daughter's participation in the program, against Columbus Recreation and Parks Department, City of Columbus, and agents, employees and volunteers. I do hereby fully release and discharge the Columbus Recreation and Parks Department, City of Columbus, and agents, employees and volunteers for any and all claims from injuries, damage, or loss which I have or which may occur to me on account of my son/daughter's participation in program. I further agree to protect, defend, and hold harmless the Columbus Recreation and Parks Department, City of Columbus, agents, employees and volunteers from any and all claims resulting or in any way associated with activities of the program. I have read and fully understand the release form. Before registration in this program is valid, this release form must be signed by the participant's parent/guardian.
