Athletic Registration Form This form is key for every league we have. We use this any many ways for our league binders for team information. Turning in this form to us is very important but does not mean your spot is held for the league you want to be in. **Holding Your Spot For Leagues** 1. Turn in FULL AMOUNT of money for the league w/ the Athletic Registration form. 2.
