RELEASE OF LIABILITY AND ASSUMPTION OF RISK

Before competing in the Kevin Honan Women's Basketball League, all participants must sign a "Release of Liability and Assumption of Risk" waiver. Please download and complete the form HERE.

 

Once completed, please email the form to Doug Chin at honanleague@gmail.com or have the completed form with you at the court on the first night of competition. 

REMINDER: You will not be able to participate without a completed form.

 

If a score is reported incorrectly on the website, please email Doug Chin at honanleague@gmail.com and the league will look into it and work to get it corrected in a timely manner.
