Coed Quickball - PreK/Kindergarten
1. 5 game league. Games will be played on Wednesday nights starting May 29th.
2. Players receive a team t-shirt and hat .
3. Registration ends May 10th.
4. Quickball program is a different version of TBall. Each week fundamentals stations of baserunning, throwing, catching and hitting will be offered, followed with a game. A LOT of instruction, learning and fun.
5. No scores kept. No stealing.
6. All games played at Mona Park.
Coed Coach Pitch - 1st/2nd Grade
1. 6 game league. Games will be played on Wednesday nights starting May 29th.
2. Players receive a team shirt and hat.
3. Registration ends May 10th. .
4. Once teams are formed, coaches may practice during the week or before the game.
5. Soft core baseballs will be used.
6. No scores kept. No stealing.

Age:
If your child will be entering 3rd grade at the beginning of the school year but would like to participate in 1st/2nd grade coach pitch, they are welcomed to sign up. Same with Quickball. If you have a child that will be in 1st grade but would rather do Quickball, please sign them up for that program. If you have any questions regarding age, please contact Kelsey at 435-253-1293.

Mona City/Mona Recreation Programs Hold Harmless, Release of Liability, Waiver of Claims, and Assumption of Risk Agreement
Mona City/Mona Recreation and its representatives make every effort to provide fun and safe recreational activities and programs. However, all recreational activities necessarily involve health and safety risks. In order for Mona City/Mona Recreation to continue to provide recreational activities and programs, participants must agree to accept all responsibility and risks for their participation. Accordingly, the undersigned hereby acknowledges and agrees as follows for themselves or on behalf of the participant:
- Participation in the Mona City/Mona Recreation activities and programs has an inherent risk of serious injury or death from a number of hazards, including but not limited to the following:
- Defective equipment or equipment failure or malfunction;
b. Interaction or collision with other persons, structures, or objects during the activity;
c. The participant’s personal negligence, including but not limited to operator error and mistakes in performing activities as directed;
d. Negligence by instructors and other persons, including but not limited to inadequate supervision, guidance, instruction, and direction;
e. Risks from outdoor activities, including but not limited to unsafe temperatures, exposure to weather and other natural phenomena, attacks or encounters with wild life, and unstable or difficult terrain;
f. Mental and emotional stress from interacting with others, losing, and facing challenges or failure in activities;
- Mona City/Mona Recreation cannot render any activity or program completely safe;
- Before beginning any exercise or recreational program, the undersigned has been advised to consult with a medical doctor as to the safety of participation;
- The undersigned, for themselves or on behalf of the participant, personal representatives, heirs, assigns, and agents, accept all risks associated with participation in Mona City/Mona Recreation activities and programs;
- The undersigned, for themselves or on behalf of the participant, personal representatives, heirs, assigns, and agents, release, waive, and discharge all claims they may have against Mona City/Mona Recreation and its representatives arising out of the undersigned’s participation in Mona City/Mona Recreation activities and programs and shall hold Mona City/Mona Recreation and its representatives harmless for all such claims, both present and future;
- The undersigned grants permission for Mona City/Mona Recreation and its representatives to authorize emergency medical treatment for the participant.
- The undersigned grants permission for Mona City/Mona Recreation to take photos and utilize those on the city’s Facebook, website or marketing materials.
COVID-19 Agreement:
1. In order for Mona City to continue to provide recreational programs and events, I agree to follow and encourage others to follow all State, County, and City guidelines and orders, which can be found online atwww.coronavirus.utah.govwhile participating in any City program.
2. Acknowledgement and Acceptance of Risks: Mona City strives to keep its facilities and programs safe and sanitary, however, I acknowledge that Mona City cannot guarantee the safety of participants, and I accept the risks involved and agree to indemnify Mona City against all claims arising out of my voluntary participation with any City program. I agree to use my best efforts to ensure the safety of all those that participate.
I HAVE READ THIS AGREEMENT IN ITS ENTIRETY, I FULLY UNDERSTAND ITS TERMS AND THAT I AM GIVING UP LEGAL RIGHTS THAT I MAY OTHERWISE HAVE BY SIGNING IT, AND I AM FREELY AND VOLUNTARILY FOR MYSELF OR FOR THE PARTICIPANT
The Policy
Full Refund Policy: Mona Recreation will offer a refund for any sports/classes if requested before the last day of registration.
1. Refund Check: A full refund will be given minus a 15% processing fee. Refund checks are issued by Mona City and may take up to 2-3 weeks to process.
2. In-house Refund (Credit): 100% refund will be credited to a participants' recreation account and will not be assessed a processing fee. An in-house credit may be utilized by anyone on the same account/family.
No Refund Policy: Once a sport/class is in process (any time after registration ends) no refund will be given.
Season Ending Injury Refund Policy: In cases of season ending illness or injury, refunds may be issued and will be prorated from the time the Rec Director is contacted with a refund request. A check refund will be assessed a 15% and may take 2-3 weeks to be issued. No fee will be assessed for an in-house credit.
Other Refund Request Policy: All other situations will be reviewed on a case by case situation.
