In an effort to protect our youth, all MYBS volunteers (board members, coaches, etc) are required to pass a criminal background screening. The cost of such screening is paid by MYBS. In consideration of being allowed to participate in MYBS program and related events and activities, the undersigned:
(1) Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably forseeable at this time.
(2) Assume all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent total disability or death.
(3) Release, waive, discharge and covenant not to sue MYBS, its administrators, directors, agents, and other volunteers of the organization, participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct MYBS activities, all of which are hereinafter referred to as "releasees,", from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise.
I grant permission to MYBS to publish photographs and editorials for the promotion of the MYBS Organization. This includes social media, MYBS website, local newspapers, and other media sources the MYBS Organization deems appropriate.
Refunds for cancellations of the program will be sent to you via check.
Personal cancellations after shirts and hats have have been ordered will be less the cost of uniform and insurance we have purchased for your child.
No refund will be returned after practice begins.
I certify that I am the parent or legal guardian of the above-named applicant. All information stated above is correct. I will adhere to all of the rules and regulations of MYBS and all other MYBS policies and rulings. I acknowledge that MYBS reserves the right to impose punishment for violations of its rules, regulations, policies and rulings, including expulsion of the participant from the program.
I acknowledge that if my player's uniform gets lost and needs replaced, it is my responsibility to work with 123 Tee's in getting a replacement uniform.
COVID 19 RETURN TO PLAY WAIVER
1. The risk to have direct or indirect contact with individuals who have been exposed to and/or diagnosed with one or more communicable diseases, including but not limited to COVID-19 or other medical
conditions, diseases, or maladies, and/or any mutation or variation thereof does exist and it is impossible to eliminate the risk that I could become infected through contact with or close proximity to an individual with a communicable disease;
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING
FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation;
and,
2. I willingly agree to comply with the stated and customary terms and conditions for participation. If,
however, I observe any unusual, significant hazard during my presence or participation, I will remove
myself from participation and bring such hazard to the attention of the nearest official immediately; and,
3. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY
RELEASE AND HOLD HARMLESS THE MORRISON YOUTH BASEBALL & SOFTBALL ORGANIZATION, its officers, officials, agents and/or employees, other participants, sponsoring agencies, directors, sponsors, advertisers, and, if applicable, owners and lessors of the premises used to conduct the event (collectively, the "Releasees"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

MYBS believes that each child who wishes to play baseball or softball should be able to do so regardless of financial circumstances. Our league will not deny any child the opportunity to play ball regardless of financial hardship. Contact us via email at

Registration will only be marked as completed once payment has been received. All children must be registered and paid for to be signed up for the season. Non-payment may result in your child not being signed up for this ball season even with the submitted form.
For those requesting financial assistance the board will respond to your request upon this registration being submitted to finalize the registration process.
