AGREEMENT REGARDING RISK OF INJURY, SICKNESS, & THE RELEASE AND WAVIER OF ALL CLAIMS 

By checking the box below of the LEGACY YOUTH SPORTS liability waiver form, I agree and understand as a participant and participants parent or legal guardian that participating in Tournament and/or League will expose me to the above normal risks of injury or harm. These risks include uneven or hazardous playing surfaces, physical contact or collisions with other players, spectators or inanimate objects on or about the court. 

I agree and understand that the sport of basketball is in itself hazardous and may result in injury to me or other players. 

I agree and understand that I have no heath or physical problems what will interfere with my participation in the Tournament/League. 

I agree that I assume all risks associated with my attendance and participation in the Tournament/League. I understand that I am and my legal guardian is solely responsible for any injuries which may occur to me as a result of my participation in the Tournament/ League and I specifically waive my right to bring litigation against the sponsors and specifically release the right which I have to assert a negligence claim against the Tournament/League sponsors, volunteers, employees their agents or representatives, and other participants. 

I agree that I hereby fully and forever release, discharge, and agree not to sue LEGACY YOUTH SPORTS, staff, volunteers, or any sponsors that may be affiliated with this Tournament/League, their officers, directors, agents, employees, representatives, and successors for any and all claims, causes of action or liability for any injury, loss or damage sustained or incurred by me arising out of or in any way associated with my attendance at or participation in the Tournament/League, including all claims, causes of action or liability arising out of the negligence of Tournament/League Sponsors, their agents or representatives. 

I agree to indemnify and hold harmless Tournament/League Sponsors, their agents or representatives from any loss, damage or expense sustained or incurred by them arising from any such claims, cause of action or liability, whether brought to me, anyone acting on my behalf, or anyone else because of conduct attributed to me. 

I grant permission to LEGACY YOUTH SPORTS, its representatives and employees the right to take photographs and/or video of me and my property in connection with Tournaments, League or other LEGACY YOUTH SPORTS sports programs. 

I authorize LEGACY YOUTH SPORTS, it assigns and transferees to copyright, use and publish the same in print and/or electronically. 

I agree that LEGACY YOUTH SPORTS may use such photographs and/or video of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content. 

I agree to grant full permission for event organizers to record any and all participation in this event through photos, video tape, recordings, or any other media, known or unknown and to use them in any manner for publicity, promotions, advertising, trade or commercial purposes, without any reimbursements of any kind due to me, or the need to pay me any fee. 

Lastly, COVID-19: The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. It is believed that an individual can be in infected with COVID-19 without their knowledge and be asymptomatic. LEGACY YOUTH SPORTS has put in place preventative measures to reduce the spread of COVID-19. However, LEGACY YOUTH SPORTS cannot guarantee that I or anyone else will not become infected with COVID-19, including my spouse, guests, unborn child, or relatives. Participation in a LEGACY YOUTH SPORTS Tournament, League, or Related event could increase the risk of contracting COVID-19. 

By checking the box below of the LEGACY YOUTH SPORTS liability waiver form, I ACKNOWLEDGE the contagious nature of COVID-19 and VOLUNTARILY ASSUME THE RISK that I may be exposed to or infected by COVID-19 by participating in a LEGACY YOUTH SPORTS athletic sports program(s), related event, or activity, and that such exposure or infection may result in personal injury, illness, permanent disability, and death to myself, my spouse, guests, unborn child, or relatives. I understand that the risk of becoming exposed to or infected by COVID-19 at a LEGACY YOUTH SPORTS program(s), related event, or activity may result from the actions, omissions, or negligence of myself or others, including, but not limited to, LEGACY YOUTH SPORTS employees, volunteers, and program participants. 

By checking the box below of the LEGACY YOUTH SPORTS liability waiver form, I understand and agree that this Release and Waiver shall be binding upon my heirs, assigns, and any personal entity action upon my behalf, including a parent, guardian, or next of kin. 

By checking the box below of the LEGACY YOUTH SPORTS liability waiver form, that I have read the above items of the Release and Wavier, understand them, agree to abide by them, and hereby acknowledge that I have read and understand this Release and Wavier.

This form is not to be copied
