I am over the age of 18 years. I understand that the 3-ON-3 Compete Tournament Staff and the facility/ staff will NOT be held liable or responsible for my injuries or any loss of property while I, the person signing this form, am competing in the tournament. I do hereby release and hold harmless the State of Delaware, Maryland, the District of Columbia, The Owners, event staff, referees, officials, and the officers, agents, attorneys, and employees from any and all liability including torts, claims and suits in law or equity for any injury, fatal or otherwise relating to or arising out of any events or incidents that occur at the 3-ON-3 Compete Basketball Tournament (hereinafter, “3-ON-3 Compete”).
By signing below, I hereby absolve the 3-ON-3 Compete and any of its owners, agents, or employees of all responsibility for injury or loss of personal property relating to the 3-ON-3 Compete Tournament.
Furthermore, I acknowledge and accept the risks involved in participating in the 3-on-3 Compete Basketball Tournament. I acknowledge that basketball can be a contact sport and injuries to myself and others can occur. I will pay, or cover through my insurance, any medical or hospital expenses, doctor bills or other expenses which could be incurred as a result of treatment given to the below named participant for illness or injury while attending or subsequent to attending the 3-on-3 Compete Tournament. I will not hold the 3-on-3 Compete Tournament responsible for any such expenses. I hereby authorize my emergency contact to act for me according to their best judgment in any emergency requiring medical attention, if I am unable to do so.
I further understand 3-on-3 Compete retains the right to use, for publicity and advertising purposes (including posts on social media sites like Twitter and Instagram), photographs, video and interviews of participants and attendees taken at the tournament. I understand and agree that participants, guests, and attendees will not be compensated for images and/or NIL used in the 3-on-3 Compete basketball tournament at any time.
As a tournament participant, I will register with my legal first and last name as written on my official state ID. As a tournament participant, I also agree to be paid either by Cash App, Zelle, (at the 3-on-3 Compete Tournament’s discretion) if my team places in any of the winning categories. The decision of the 3-on-3 Compete Tournament regarding who has won the games shall be final and non-appealable.
As a tournament participant, I have been informed of and agree with the tournament entry fee, the tournament rules, the tournament information sheet, and the prize pool winnings.
I also understand and agree that fighting, striking, hitting, slapping, kicking, spitting on or otherwise assaulting another tournament player or anyone involved in the tournament will result in an immediate forfeit of all games of my team, my immediate expulsion from the facility, and a forfeit of the tournament entry fee of the team. I acknowledge that if I take such an action, I will
also be escorted off the premises immediately.
As a tournament participant, I also acknowledge that 3-on-3 Compete will assure the privacy of it's members, employees' and visitors information. We prohibit, photography, filming, and recording of any type with any professional equipment. Participants are only allowed to use a cell phone to take pictures to record video and voice, while participating in the 3-on-3 Compete Tournament. Any violation of this policy during the tournament will result in legal action from 3-on-3 Compete.
